Who should take the test?

Cancer of the prostate gland typically occurs in older patients. Prostate cancer is among the most commonly diagnosed malignancies in men and is the third leading cause of cancer-related death. It usually originates from the external prostate glands, grows slowly, and tends to metastasize (=subsidiary tumors in other parts of the body) to lymph nodes and bones. If diagnosed early, prostate cancer has a good chance of being cured. Monitoring PSA levels can help diagnose cancer early and therefore lead to a timely and appropriate treatment.

Preventive measures to minimize the risk of prostate cancer are recommended at about 45 years of age. If cases of prostate cancer have occurred in the family before, preventive tests are advisable at earlier stage in life (around 35).

PSA is a protein formed in the prostate and is released in the seminal fluid. The protein PSA can also be detected in the blood, but only in a concentration that is significantly lower when compared to the seminal fluid. If malignant cells occur in the prostate, they often produce more of the protein. For this reason, PSA determination has become very important in the early detection and follow-up of prostate cancer. For the PSA test, only a small blood sample is needed.

What is a PSA test, and why is it important?

The PSA test is a blood test that measures the level of a protein called prostate-specific antigen in the blood. It's important because elevated PSA levels can be a sign of prostate cancer.

What can cause increased PSA levels?

The level of PSA depends on numerous factors: First, how much PSA is released from the prostate into the blood varies. Then the amount of prostate tissue (both benign and malignant) influences the level. Thus, it increases with prostate volume, usually with age because of natural increasing prostate enlargement. Other diseases and irritations of the prostate can also increase the PSA level (especially the free PSA), but usually only for a short time: for example, prostatitis (inflammation of the prostate), acute urinary retention (ischuria), sampling (prostate biopsy), prostate massage and rectal examinations. Activities such as sports (e.g. cycling, horseback riding) or sexual intercourse may slightly increase the PSA level.

What should I do when my PSA level is high?

It is important that you have a medical examination if your PSA level is high. In many cases, the PSA value can be elevated without a cancer being present. Doctors can investigate this more closely and, depending on the diagnosis, give you individual recommendations for therapy.

How often should PSA testing be done?

The frequency of PSA testing depends on individual risk factors and your physician's recommendations. Typically, annual testing is recommended for men over 50, and earlier or more frequent testing may be advised for those with higher risk factors.

I am not getting enough blood from my finger, what can I do?

Ensure optimal conditions for the blood draw: Hold your hand under warm water for an extended period and gently rotate your arm to enhance blood circulation. When pricking the finger, aim for the side rather than the direct top of the fingertip. Typically, the middle or ring finger of the non-dominant hand is most suitable. While piercing, place your finger on a stable surface to prevent reflexive withdrawal. Applying a gentle squeeze and massaging towards the fingertip can facilitate blood flow.
